Each family who fills out this form will receive a free directory.
(Directories to be distributed with second (9 week) grades in late October.)

The FHS PTSO creates a school directory to be given to each family included in the directory. The directory is a combination of school information and names, addresses, and telephone numbers of the students. Your permission is needed for inclusion in the directory. With this in mind, please fill out the following information and sign, thus giving permission for your student(s) to be in the directory. Only ONE FORM need be filled out per family. Please include all FHS students in your family on the form.
